Any prior MRI, X-ray or ultrasound imaging — the discs or the report — plus a short history of what you’ve already tried. Nothing else is required.
No. The care here is above the standard of care insurance will authorize, and working within it delays appropriate treatment. Pricing is transparent and given upfront — you’ll know the cost before anything is scheduled. Many patients are able to submit to their insurer for partial reimbursement after visits and interventions — for example evaluations, ultrasound, diagnostic injections and energy-modality codes.
